The process to receive a Stock Certificate Refund is submitting by one of the options below:
- Mailing original stock certificate to Lindgate
- Attaching a digital copy of your stock certificate below
- Complete the “Lost Stock Certificate Affidavit” below
Once the stock certificate is received, the refund will be issued, unless you choose to kindly donate the refund.
If you decide at a later date you’d like to rejoin the pool you will be placed at the bottom of the waitlist.
